Patient Advocacy Program

AdvocatesWe're Here to Help, No Matter Where You Are

We're not just in our clients' homes. We're by their side in the hospital making sure they get the attention they need, and accompanying them to doctors' appointments. Our staff is with them during outpatient treatments such as dialysis, chemotherapy, and physical therapy. We are part of the conversations with their pharmacists about medications to be sure our clients understand what they're taking and why. We're the expert eyes and ears that keep our clients' health and quality of life as our number one priorities.

Our Role as Patient Advocates

In the midst of a health crisis, it's easy to feel overwhelmed. Will you ask the right questions? Will you choose the right doctor or facility? Will you or the one you love receive the focused attention required? Are you equipped and prepared to manage health care at home?

In addition to providing expert nursing care, we are your supportive advocates, working on your behalf with physicians, pharmacists, and even your family to ensure that your best interests are always the top priority.

As advocates, we will:

  • Sit in on doctors' appointments to ask the important questions, and keep track of the answers
  • Review your list of medications to make sure it's complete and accurate
  • Coordinate the efforts of various health providers, from physical therapists to dialysis nurses, to be sure that everyone is on the same page and understands your health goals
  • Respect your confidentiality and privacy, and communicate only what you want others to know
  • Serve as a contact point to extended family and friends for progress updates, if you decide you want them to know
  • Join you in the hospital to review your care regimen and charts, and speak with medical staff about concerns you may have, ever vigilant no matter how long your stay to ensure you're receiving the best possible care
  • Accompany you anywhere, anytime, whenever your healthcare is being discussed or you're undergoing treatment, including doctors' appointments, diagnostic testing, outpatient therapy or procedures, as well as in-home care
  • Schedule appointments, provide updates to physicians, and coordinate med schedules
  • We stay completely focused on you!
